How to use it
Record the ECOG performance status in every letter where lung cancer is even a vague possibility. Patients get discussed at a regional lung cancer MDT on the strength of the letter alone, and surgeons and oncologists cannot plan treatment without it — a missing performance status sends the referral back and costs the patient a fortnight. Fill the smoking lines even when the answer is never: never is a finding in a breathless patient, and the fields for very brief advice and referral exist so that what you offered is in the record rather than in your memory. The birds, pets, travel and occupation lines are four seconds of dictation that occasionally rewrite the whole diagnosis.

- ECOG performance status (Oken et al, Am J Clin Oncol 1982) The scale itself, unchanged since 1982 and still the one every lung cancer MDT, trial protocol and systemic anticancer therapy decision runs on.
- NICE NG122 - Lung cancer: diagnosis and management Performance status drives what treatment can be offered, so it belongs in the first letter rather than the third.
- NICE NG209 - Tobacco: preventing uptake, promoting quitting Very brief advice at every contact, with an offer of treatment and referral rather than advice alone. The three fields make the offer auditable.
- ATS/ERS technical standard on spirometry (Graham et al, 2019) Report FEV1, FVC and the ratio with per cent predicted, so the numbers can be compared with the ones in the last letter.
